Stor-Stat and Photo-Stat, LP is a regional medical record copying service which has been processing requests for medical records since 1996. A HIPAA-compliant authorization form must be completed and signed by you in order to either have your records sent to another physician or to you. FAX documents can still be received at 866-370-0570 but we cannot accept FAX requests for Release of Information. Prescription refill requests should ONLY be sent electronically. Those sent via FAX may only be viewed twice per week. No guarantee is made regarding FAX receipt. If critical, regarding administrative matters, please use the US Postage Service and correspond to the address above.
